Part I – The Symptoms Imagine that you work at the medical examiner’s offi ce for a major metropolitan city. As Chief Medical Offi cer, you investigate suspicious deaths and provide toxicology services for the county. Unfortunately, it’s been a busy week. In the past fi ve days, seven people have died, all with similar symptoms. It is your job to examine the data and determine the cause of death for these victims. Th e fi rst was a 12-year-old girl. Her parents said that she was awake in the middle of the night complaining of a stuff y nose and sore throat. Th ey gave her an extra strength Tylenol and sent her back to bed. At 7am the next morning, the parents discovered that the girl had collapsed on the bathroom fl oor. An ambulance rushed the girl to a nearby hospital, where she was pronounced dead. Th at same day, paramedics found the second victim unconscious on his kitchen fl oor after what they thought was an apparent heart attack. Sadly, the victim’s brother and fi ancée also collapsed later that night while the family gathered to mourn his passing. Both had taken Tylenol to help them cope with their loss shortly before collapsing; neither survived. In the next four days, four other similar deaths were reported, all in the same neighborhood and all with similar symptoms. Are these seven deaths related? What is causing these people to die? It is your job to answer these questions before more deaths are reported. Symptoms exhibited by most patients: • Dizziness • Confusion • Headache • Shortness of breath/rapid breathing • Vomiting Most deaths were very rapid, occurring within a few hours of symptoms. Questions 1. Are there any similarities or connections between these seven individuals? What questions would you want to ask the families to answer these questions? 2. In your opinion, are these seven deaths connected? Why or why not?

Part II – Autopsy Report • Immediate cause of death was hypoxia (suff ocation or lack of oxygen). • Tissue sections from heart, lung, kidney, and liver all show massive cell death. • Staining with specifi c dyes showed major mitochondrial damage within the aff ected tissues. • Oxygen levels in the patients’ blood were approximately 110 mm Hg (normal range is 75 – 100 mm Hg). Questions 1. Recalling your knowledge of the function of organelles, what function of the cells was interrupted in these patients? Could this loss of function lead to the death of these individuals? Why or why not? 2. Given the data in the autopsy, were there any reports that seemed inconsistent with the immediate cause of death?

Part III – Subcellular Metabolite Analysis Detailed analysis of the damaged cells showed that ATP levels in the mitochondria were very low. Levels of pyruvate and acetyl coenzyme A (CoA) were normal. You begin to suspect a malfunction of a specifi c cellular metabolic pathway and so you request a more detailed analysis of the sub-cellular components of the aff ected cells from the autopsy. Th e levels of key metabolites are reported below:

Part IV – Role of Cyanide You are now convinced that you know the cause of death for these victims and quickly report it back to the police as this is a very dangerous situation. After realizing that the electron transport chain was no longer functioning, you started to suspect poisoning and ran a blood test for various poisons that you knew aff ected the electron transport chain. Th e test of all seven patients came back positive for cyanide. Cyanide irreversibly binds to cytochrome c oxidase (CcOX) of the electron transport chain and prevents the transfer of electrons to oxygen, the fi nal electron acceptor. Questions 1. What aff ect would cyanide have on the electron transport chain and the production of ATP? Explain your answer. 2. Given what you now know about the action of cyanide on cellular respiration, explain why the patients died of lack of oxygen while their blood oxygen levels were normal? 3. Would artifi cial respiration or oxygenation have saved these people? Why or why not? 4. Looking back at the information you have about the people before they got sick, can you suggest a possible source of the cyanide poisoning? 1) Summarize, in your own wording, what the APA Ethics Code says on confidentiality, disclosures, and informed consent.
2) Review and cite the laws for your state with regards to mandated reporting including if applicable, abuse and neglect, the duty to protect and/or warn e.g. Tarasoff Warning, suicidal behavior, sexting by minors and viewing child pornography.
3) Assuming you were establishing a therapy practice in your state, you will be providing all clients an informed consent for treatment. In addition, during your first session, you decide that it is important to review with them in person, the limits of confidentiality. How would you explain to clients that confidentiality is not absolute? Write a 1 to 2 paragraph introductory monologue in which you explain to your client the limits of confidentiality listing some specific examples as to when confidentiality may be broken.
4) For each of the 5 scenarios below, provide the action you need to take based on your state laws and regulations. Make certain you cite the regulation. If a report is not mandated by the state, indicate that and explain what you might do.
(A)Your 17-year-old male client is involved in a consensual sexual relationship with a 17-year-old male classmate. In responding to this scenario, you will have to whether your state does or does not differentiate between different types of sexual activities.
(B) Your adult client has told you that if he were not such a tolerant individual, he would kill his mother-in-law the next time he saw her.
(C) You usually have snacks for your 10-year-old client, but you were unable to replenish your supply. He is disappointed and tells you that this just like his home with no food in the cupboards or the refrigerator
(D)Your 12-year-old female client tells you that she is pregnant and wants to terminate the pregnancy. Neither you nor her parents were aware that she even had a boyfriend.
(E) Your adult female client arrives at the session with bruises partially covered by make-up and long sleeves on a hot day. She starts crying at the beginning of the session but refuses to discuss the bruises.

There are many reasons that employees might feel that they are discriminated against when applying for positions. If the employee is not offered the position, they may believe that this has to do with their status within a protected class. When candidates fall within a protected class based on race, sex, disability, age, national origin, or religion, the need for employers to understand and follow the “Uniform Guidelines on Employee Selection Procedures,†a procedural document published in the Federal Register to help employers avoid discriminatory action, is high. Your responses to the discussion questions should relate the assigned readings and viewings to the discussion topic and should not consist only of personal opinion. Your opinion is important to the discussion, but it is also important that you add connections to the material to support your views. Without a foundation in the literature, your opinion has little professional value. For example, “Based on Trevino and Nelson’s stance that cultural relativism creates an environment where unethical behavior is justified through the cultural discourse (year, P#), the discussion should center around……” This type of statement allows you to connect your views to the literature.
